In addition copies of blood work lab results and results for HIV, Hepatitis A,B,C and Tuberculosis are required. Monkeypox test is required, for DC applications it can be substituted with a letter from Doctos stating no sighs of infection detected and further testing not needed, letter also should be stamped and signed. The medical form is valid for 90 days from test date.. Children under the age of 3 will need to provide vaccination records instead.
